Sarah returns to Waterbury with her husband and journalist Dylan Bennett to stay at a house bought from realtor Robin Turner. The house is the same one where Sarah's late parents Bryan and Rachel Ingram were murdered before her birth on Halloween by Tom Winston in 1988. Winston had concealed his identity with a medieval robe costume and a leather hood and had been dubbed "The Executioner". The second Executioner surfaces for the first time, chasing Sarah down her street that night. Following a suggestion from Tom after their first meeting, Sarah discovers a camcorder and illicit sex tapes her parents buried in their house, tapes that her neighbor Verna McBride had discovered and taken to her house. The Executioner ties down and dismembers Verna in her bedroom. During the second visit, Tom reveals to Sarah that the killer is targeting his victims who violated one of the seven deadly sins and that Verna's death of dismemberment was the biblical punishment for wrath.

Sarah's grandmother, Brenda arrives at the house to stay with the couple until the killer is caught. While setting up her art gallery store, Sarah receives a package containing a severed human finger and a note; following Tom's suggestion that the note contains geographic coordinates, she discovers the remains of Verna's husband Peter in the woods and that his wife murdered him out of anger at his illicit affair with Rachel from one of her tapes. Lured out into an alley way by a severed power cable, Robin is attacked by the Executioner who injures him before he is rescued by a bystander. While Robin is recovering at the hospital, police deputy Cam Henry realizes that Robin's husband Justin is the killer's next victim. He attempts to report police chief Iain Vaughn at the house party but Justin dies after ingesting cocaine laced with rat poison, serving as the punishment for his gluttony. The police arrest Heather Peterson who made verbal threats against Justin and Robin earlier, under suspicion of the current murders.

Robin discovers from a lawyer that Justin had bribed money from his clients, and that he stole their home from a family who died from oxygen toxicity from the house's propane heater after Justin moved them to another house without electricity or heating. As Sarah is driving out of town with Brenda, they swerve off the road to avoid a passing pickup truck and are forced to walk back to town. In the woods, Brenda confesses her role in Ada Pavlovik's accident as a teen when she attempted to drop a cinder block on her former best friend Sonja who was driving in the car after her brief relationship with the town mayor Ronald Edwards. Fearing for her grandmother's safety, Sarah is separated from the latter who attempts to kill the Executioner, but he captures her, ties her to a cinder block and tosses it into the lake at a nearby boathouse. Brenda's drowning death, the punishment for her envy, prompts the police to release Heather from custody.

The police verify that the truck that ran off Sarah and Brenda belonged to Verna's nephew Trent. However, the latter is chased into a snake pit in the woods where he is killed by the Executioner as punishment for his sloth. Dylan interviews Tom in prison, under request from his boss Alison Sutherland and New York journalist Lisa-Ann Follows. Tom provides evidence to Dylan that suggests he has had a greater interest in Tom than he let on to Sarah. Cam's wife June confides to Sarah that she and Trent had an affair while they were EMT partners; she also explains to Sarah about Trent's encounter with Ariel Peterson, Heather's missing daughter who disappeared five years ago. But when Sarah observes the security camera footage, June was also involved with Trent before Ariel's disappearance. After comforting a hysterical Heather in the street, June is abducted at the church by the Executioner. She awakens in a cornfield naked, paralyzed with lidocaine attached to an IV, and covered in honey which attracts the animals that eat her alive.

Cam and Sarah argue over suspicions each other have about the killer. Alison records her requested on-air interview with the Executioner who, in full costume, obliges at an abandoned foundry to publish the footage on live television. At the bar, she confides to Lisa-Ann that she fabricated evidence that tied Benny Peterson to his daughter's disappearance until the town held him responsible for the incident, driving him to suicide. Sarah and Robin believe Cam's father and local pastor Alan is the killer. The next day before heading to New York, Alison is abducted in her limo by the Executioner who takes her back to the foundry and decapitates her for her greed; her head is later found in the local restaurant's deep fryer. After acquiring the digital files of her parents' remaining sex tapes from Ronald, Sarah discovers that one of the videos featured a young Tom Winston. Meanwhile, Vaughn heads home to his basement where he has concealed Ariel inside with her son Jake.

Five years earlier, Chief Vaughn abducts Ariel after June and Trent left her behind on the road, and rapes her. After suspecting Sarah's attempts with Cam to interrogate Marjorie Travers about Ariel's whereabouts, Vaughn kills Marjorie in her apartment. He then captures Sarah in his car, who revealed Alison's fraud of framing Benny for Ariel's disappearance and Marjorie's deception. Vaughn takes her to a junkyard and attempts to kill her and make it appear as the Executioner has killed her for her pride—the punishment being to be "broken on a wheel" - but Sarah manages to escape, exposing Vaughn's sin of lust. Enraged, he attempts to punish himself by self-immolation along with Ariel and Jake but spares them until the police arrive and reunite them with Heather. Tom insists to Sarah that he is her birth father until she demands his reason for murdering Bryan and Rachel. Meanwhile, Vaughn escapes to a secluded lakeside shack where the Executioner knocks him out and traps him in a crematorium where he is burned alive.

At the police station, Dan is informed of his daughter's death while Hanson interrogates Wyatt who nonchalantly claims that he didn't murder the other tenants who were complicit in Kit's death. After arriving at the summer solstice party, Saadia becomes anxious and decided to leave afterwards, recounting her past to Connor of being the lone survivor during a civil war in Afghanistan after her grandparents and relatives were executed by the military. Meanwhile, Charlie, a student who constantly bullied Jen and Saadia, is killed by the Druid inside the girls' washroom. While reviewing the previous crime scenes, Hanson deduces that the current murders are devious and done on purpose and may not be connected to Kit's murder. Back at the apartment building, the Druid, who has been manipulating the other residents into their deaths, lures both Dan and Angel into the basement during their confrontation and released a gas from the vent that knocks them unconscious. Angel and Dan later awaken with their faces glued together.

In 19th century Toronto; a murder of an upper-crust at the alleyways of Devil's Elbow, under authority of tycoon Basil Garvey, is investigated by newly appointed police detective Kenneth Rijkers. From his interview to procurer Horatio Dixon and his girlfriend Daisy Zywiecki, the figure was wearing a widow's funeral clothing in black; hence to the killer's name. During the victim's second autopsy with medical examiner Dr. Melanda Israel, Rijkers found a hidden note from the corpse and that the killer is preying on the targets connected to the murder of Margaret Mehar. With the help of the current victim's wife Regina Simcoe, he discovered pieces of evidence relating to Margaret and the infamous "Jack the Ripper" hoax. At Devil's Elbow, Garvey's henchmen led by Eddie Jacobs assaulted Horatio and the other street workers until Jacobs unwittingly kills Daisy in self-defense before they flee. As Horatio mourns over her death, the Widow appears at the street and slowly decapitates him.

Rijkers arrested Jacobs earlier under suspicion of the killings but was released earlier by his boss Isaac Kashtinsky, who pulls Rijkers out in investigating the murder case. When local newspaper editor Enid Jenkins was confronted earlier by Garvey who cautioned her to discard the article of the Widow's attack, she sought help from brothel-keeper Terrence Crenshaw to take down Garvey but he refuses in fear of getting involved until his cabaret performer Salomé gave the evidence to her containing photos of Garvey's scandal. Before she could be able to publish the paper at her shop, the Widow attacks Jenkins who had falsified the father of current town pastor Andrew May Jr. as Margaret's murderer before he was executed twelve years ago; given by Garvey in exchange for her promotion. After Jenkins revealed that Garvey was behind Margaret's death and orchestrated the extortion to conceal his scheme, she is killed after the killer throws her into the printing press that crushes her to death.

While observing Jenkins' murder, Rijkers revisits Regina to Alistair's office and uncovered the photos of Garvey and the other remaining targets of the killer from a party the week before Margeret is killed; the other victims are Jacobs, Kashtinsky, Salomé, Israel, the Botticelli sisters, and local magician Georges Rondeau. At the theater after rescuing Jacobs during the latter's earlier confrontation with Rondeau, Rijkers pursues the Widow through the street and was beaten during their fight but the killer spares his life from murdering him. Jacobs, now guilty of his involvement at the ordeal for not helping Margaret from Garvey and his cohorts whom they tortured her to death earlier, attempts to leave town at night after standing up against his employer from the abuse. En route, the Widow chases after him and dislodges the carriage off the road. Jacobs attempts to run away, but the Widow trips him with the carriage's reins and incapacitates him with a sledgehammer before using it to smash his head.

The Widow carved Jacobs' corpse and left it into the local butcher shop to be bought by Garvey's housekerper Gladys and baked into individual meat pies at his dinner party to agitate him and his companions. Rijkers arrested Rondeau at Regina's residence for the alleged murders. When Rondeau denied their allegations to him as the killer; Kashtinsky, who was overdosed with cocaine, tortures him at his jail cell to confess until he is stopped by Terrence and Rijkers who sent the officer away for his recovery while releasing Rondeau. At nighttime, a drunken Kashtinsky became disoriented and saw visions of Margeret; remembering his first meeting with her at the Botticelli's house party twelve years ago and she rejected his sexual advances when he tried to propose her until she is captured by Garvey. Wandering into the streets, Kashtinsky is lured into an empty carriage by the Widow who ties him to a runaway horse that violently drags him though the town.

Salomé, before being taken to custody care by Terrence, was a former prostitute under Horatio alongside Margaret. Enabled by Terrence's sympathy, Salomé gained personal leisure as co-worker of his brothel while Margaret was sent to work as a house servant for the Botticellis. After Margaret humiliated her at the party, Salomé conspired with Garvey and the others to lure her into the bondage room at the brothel and brought Alistair to rape her until Garvey killed her. The past events have strained Salomé at the current time when, after being interrogated by Rijkers, she killed Terrence during their heated argument at his confrontation upon learning that Venetia and Viviana Botticelli bribed her in selling their stepsister Verdi into sex slavery earlier. Now alone in the house while attempting to evade imprisonment, a grief-stricken Salomé saw the Widow at the hallways and tried to escape but the killer captures her to the bondage room and sedated her body with anesthetic before dismembering her alive with a bonesaw.

Now feared of being targeted by the Widow's killing spree, Israel finally confessed to Rijkers of Garvey's crime and her role in Margaret's murder. Rijkers, who now assume position to run the police station, apprehends Israel for interrogation; including Garvey and Rondeau. But while the process is complicated to resolve Margaret's death and lack any sufficient evidence to convict Garvey, Rijkers' discussion was interrupted when Regina pleads for Israel to save Andrew after he was stabbed at the street by Garvey's henchman. After being confronted by the Widow who then left the hospital, Israel succeeded the operation and delivered her blood to Andrew. During the night, she revealed to Rijkers that after Margaret died, Garvey paid her to mutilate her corpse and left it outside the alleyways for Jenkins to frame Andrew's father. When Rijkers moderately released her and Rondeau from incarceration, the Widow reappears and straps Israel into her operating table; pinned with conductors throughout her body and electrocuting her.

Verdi, having been freed from the abuse of her stepsisters, is bought by Garvey from Venetia and plans to propose his marriage to her. Plagued by his sanity and the events since being associated to Garvey's felony, Rondeau is prepping for his last magic show tonight after his assistant Shanika left town in fear from the Widow's murder spree. Manipulated by Garvey to entice her revenge against her sisters, Verdi volunteered Venetia to perform at Rondeau's show and rigged his trick during their performance as he unwittingly sawn Venetia in half. Viviana is arrested and imprisoned to jail by Rijkers after Verdi had framed her in perpetrating her sister's death. This was part of Garvey's motive in taking out one of the Botticellis as the killer to avoid capture and conserve his reputation. Rondeau is then attacked and killed by the Widow who strangled him to death at his tent. Meanwhile as Rijkers spent the night at her house, Regina is revealed to be the Widow after sustaining an injury to her wrist from him during their second encounter earlier.
